This system was first introduced in Mists of Pandaria, and continued with Warlords of Draenor. It's a simple method to increase the quality of your equipment, which lets you to avoid the tedious grind and avoid a bunch wipes in Mythicand Dungeons.
When an item is upgraded the type of item it is upgraded to. The upgrade of a normal item creates an Exceptional item, while upgrading an item that is Exceptional will result in an elite version of the same base item. The item will retain its pre-upgrade stats, and will gain an extra bonus from its new base type. This bonus is randomly chosen. It could be a weapon that has an increased damage range, an improved speed of reloading, or enhanced max defense roll.
The amount of extras you can get when upgrading depends on the number of upgrades performed. Each upgrade will boost the extras by up to 3 points. In addition, each upgrade will also boost the stat that is used to determine the main one by 1.
Some upgrades also give you an additional random boost in addition to stat boosts. These boosts can be useful and should be taken into consideration when deciding what items to upgrade. Some of these boosts are only suitable for specific builds (such Whistan's shield, for FB sorcs) while others could provide significant value to specific collectors.
Some items come with built-in boos that is not beneficial when upgraded. For instance the Goldwrap offers an additional potion slots. These are often upgraded to add value to a close-to-perfect or perfect item.
Costs
There are some costs related to upgrades of items. The first is the cost of performing the upgrade. It can be costly and is dependent on the new level of the item as well as the number perks it gains. It is also costly to upgrade an
item upgrading to a higher level (such as Explorer > Champion, or Veteran > Heroic).
The other is the cost of items needed to upgrade. The items used for upgrades are usually either Power Dices (dropped by monsters 21and over) or Minerals and Erons (obtainable through completing specific Mists of Pandaria or Warlords of Draenor quests). The quantity of these items required can be seen in the inventory of an upgrade vendor.
It takes time to upgrade an item. Depending on the size of the
upgrade item, it could take as long as 8 hours. The base stats of an upgraded item are calculated based upon its new ilvl as well as other characteristics. This calculation determines the new Armor Class bonus, damage dice, as well as other important statistics. In some cases the upgrade will provide additional advantages that can be picked from a list of available perks.
Rarely, the new advantages acquired will cost more than one perk slot to acquire In these instances, the upgrade will only replace the perks with the highest cost. The upgrader can also make other modifications to the item, such as changing its weapon damage type or eliminating an operational or unwieldy option. The cost for these changes is determined by the item's current ilvl and it may require a special material like adamantine alloy.
Success Rate
Item upgrades are based on an opportunity, and like any random system there is the possibility that the upgrade will fail. However there are a few things that can increase the chances of success. This includes using an Upgrade Pity or adding extra materials to your upgrade.
The Upgrade Pity can be purchased through the NPC for smithing in Lost Ark. It increases the odds of an upgrade's success by an amount. This is similar to the luck bonus certain items offer however it is more solid. It can be added to any weapon or armor and can be used up to three times.
You can also add additional materials to the upgrade to increase the chances of success. These are shown in a window above the + button. There are some limitations. The maximum amount of material you can upload is shown directly above the + button.
Another method to increase the chances of an upgrade succeeding is by using the item known as Blacksmith Blessing or an upgraded version of it. This item is helpful when you own an item or weapon gear that has reached the maximum level for its tier or is close to it. It allows you to upgrade that piece of gear to the next level without having to worry about it breaking.
If an upgrade is unsuccessful the system will be restored to its original state. This is typically a break, but it could also be a downgrade. Save an upgrade until you're close to the maximum level for an
item level upgrade.
Kafra offers a variety of products that can be used to increase the chances of an upgrade being successful. The higher the rank of the item, the more Zeny and items will be needed to upgrade it.

To perform a normal upgrade, you must first have the appropriate number of cores. Click the button next the item you wish to upgrade after you have the cores. The upgrade window will show a percentage indication of your chance of success. The percentage is based on the upgrade level and core nature of the item you have selected.
You can also choose the option of a security feature for your item which will safeguard the item in case of failure. The cost of this protection can vary but will always be lower than the cost of purchasing a new item. The protection is only valid once for each upgrade and will last until it is used again.
The interface for the item upgrader may be confusing, but it is not impossible to navigate. The main menu is located on the left and the upgrade window is located on the right. The arrows on the left of the UI indicate the different stages in the upgrade process. If you're not certain what you're doing, use the question mark icon for help. The upgrade process can take up to five minutes.
